Here’s How It Works
Enroll in both dental and vision coverage from Ameritas during open enrollment. Select your dental plan of choice and choose which eye care network you prefer. When you visit a provider, submit your claim online at Ameritas.com. If you visit an in-network provider, benefits will be issued to that provider. For out-of-network providers, benefits may be issued to you or the provider (as selected on the claim form).
